How sprawl adds to this
A village-edge estate can be asked for a footpath. It cannot be asked for a health centre the NHS has no capital pot to build. That is why the list grows and the building never appears.
What a larger settlement can do
City-scale land value can pay for health buildings with the homes. That is the fund the 150-home estate does not have. Concentration is how the surgery is paid for. Scattering is how it is promised and never built.
The pattern is fewer, larger places with infrastructure first, not another estate on the edge of every village.
