How sprawl adds to this
The estate arrives. The list gets longer. There is no mechanism that opens a surgery when the houses do. Another permission on the same town is another year of the same wait.
What a larger settlement can do
New health facilities in a larger planned place help Haverhill only if they are nearby and open to the town. Concentration can fund the building. It does not automatically shorten Haverhill's list. What it does stop is another estate with no surgery attached.
The pattern is fewer, larger places with infrastructure first, not another estate on the edge of every village.
